KEKSA BEMORLARDA YURAK YETISHMOVCHILIGINI ANIQLASHGA OID PROTEOMIK PROFILLASH

Abstrak
Maqsad. Ushbu sakson oqsil ilgari yurak-qon tomir patologiyasi bilan bog'liq deb aniqlangan bo'lib, ushbu tadqiqot ishining maqsadi yurak yetishmovchiligi yoki umuman yurak yetishmovchiligining yangi xavf omillarini topish uchun mazkur oqsillarni proteomik profillashdan foydalanishdir. Materiallar va usullar. Yaqinlikni kengaytirish tahlili (proteomik profillash) yurak yetishmovchiligi bo'lmagan holatda boshlang'ich bosqichda keksa yoshdagi turli kogortalarga ega ikki xil jamoada o'tkazildi. Prospektiv tadqiqotlar sifatida Qon tomir tizimini prospektiv o'rganish (PIV) va Katta yoshli erkaklarni longitudinal o'rganish (LSAM) tanlangan bo'lib, ularning mos qiymatlari (80 hodisa, yoshning medianasi = 70,2 yil, n = 901) va (90 hodisa, yoshning medianasi = 77,8 yil, n = 685) ni tashkil etdi. PIV da jins va yoshga tuzatish kiritilgan holda kuzatilganidek, yurak yetishmovchiligining yuzaga kelishi 29 ta oqsil bilan bog'liq edi. Tuzatishlar ko'p sonli testlardan so'ng kiritildi. LSAM da yurak yetishmovchiligi bilan bog'liq 18 ta oqsil aniqlandi. Har ikkala kogortada 9 ta oqsilning yuqori darajasi yurak yetishmovchiligining yuzaga kelishi bilan bog'liq bo'ldi. Belgilangan xavf omillariga qo'shimcha tuzatishlar kiritildi. Chap qorincha sistolik funksiyasining yetishmovchiligi aniqlanib, u o'sish differensiatsiyasi omili 15 (GDF-15), urokinaza turidagi plazminogen aktivatorining yuza retseptori (U-PAR), matriks metalloproteinaza-12 (MMP-12), o'sma nekrozi omili bilan bog'liq apoptozni keltirib chiqaruvchi ligand retseptori 2 (TRAIL-R2), spondin-1 (SPON1) va follistatin (FS) mavjudligi bilan bog'liq ekani ko'rsatildi. Bu exokardiografik kuzatuv orqali aniqlandi. Barcha P qiymatlari 0,02 dan past edi. Natijalar. PIV ning yakuniy tavsiflari jami 901 nafar ishtirokchi orasida yoshning medianasi 70 yil ekanini o'z ichiga oldi. LSAM da tavsif 685 nafar ishtirokchi orasida yoshning medianasi 78 yil ekanini ko'rsatdi. PIV da kuzatuv davrida yosh oralig'i 0,1-10,9 yilni tashkil etib, medianasi 10 yil bo'ldi, yurak yetishmovchiligi sababli 80 marta kasalxonaga yotqizish qayd etildi, yurak yetishmovchiligi darajasi esa 100 kishiga 0,96 ni tashkil qildi. LSAM da yurak yetishmovchiligi sababli 90 marta kasalxonaga yotqizish bo'lib, yosh oralig'i 0,2-10,9 yilni, medianasi 8 yilni tashkil etdi, yurak yetishmovchiligi darajasi esa 100 kishiga 1,83 ni tashkil qildi. PIV da soxta topilmalar darajasi 5% bo'lib, jins va yoshga tuzatish kiritilgandan so'ng 29 ta oqsil bilan bog'liqlik aniqlandi. LSAM da 18 ta oqsilning yurak yetishmovchiligi yuzaga kelishi bilan bog'liqligi nominal edi. Xulosa. Keksalar orasida o'tkazilgan ushbu tadqiqotda proteomik profillash orqali oqsillarning fibrinoliz, apoptoz, yallig'lanish, matriksni qayta shakllantirish va yurak yetishmovchiligidagi ishtirokiga oid bir qancha yangi bog'liqliklar aniqlandi. Ushbu tadqiqot natijalari asosiy mexanizmlar va klinik qo'llanilishni o'rgangan boshqa tadqiqotlarga ham mos keladi.
