RESURSLARI CHEKLANGAN SHAROITDA SURUNKALI MIELOID LEYKOZ BILAN OG‘RIGAN BEMORLARNING DEMOGRAFIK VA KLINIK PROFILI: HAR TOMONLAMA TAHLIL

Abstrak
Maqsad: Surunkali mieloid leykoz (CML) global sog‘liqni saqlash yuki hisoblanadi, ayniqsa zamonaviy davolash imkoniyati cheklangan, resurslari kam hududlarda. Ushbu tadqiqotda bunday sharoitlardagi CML bilan og‘rigan bemorlarning demografik va klinik xususiyatlari keng qamrovli tahlil qilinadi. Biz [insert data source(s)] dan foydalanib, CML tarqalishi, yosh bo‘yicha taqsimot, kasallikning namoyon bo‘lishi, davolash tendensiyalari va natijalarini tahlil qilamiz. Shuningdek, sog‘liqni saqlash muassasalari cheklangan resurslar sharoitida CML ni qanday boshqarishini o‘rganamiz. Ushbu tadqiqot resurslari cheklangan sharoitlarda CML ning murakkab manzarasini yoritib berish orqali shaxsiylashtirilgan terapiyalarga turtki berish hamda bemorlar natijalarini yaxshilash uchun sog‘liqni saqlash resurslarini taqsimlashni maksimal darajada samarali qilishni maqsad qiladi. Metodologiya: 410 nafar CML bilan og‘rigan bemorning demografik va klinik ma’lumotlari baholandi. Yosh bo‘yicha taqsimot, jins, hududiy vakillik, klinik ma’lumotlar (taloq o‘lchami, WBC soni, gemoglobin darajasi, trombotsitlar soni), kasallik fazasi, molekulyar tadqiqotlar (PCR, FISH), xavf stratifikatsiyasi, davolash usullari va bemorlarning natijalari inobatga olindi. Natijalar: Demografik ko‘rsatkichlar: 410 nafar CML bemorining o‘rtacha yoshi 38.31 yosh bo‘lib, 11 dan 80 gacha oraliqda bo‘ldi. Yosh bo‘yicha taqsimot 31-50 yosh guruhining sezilarli ishtirokini (53.41%) ko‘rsatdi, undan keyin 12-30 (29.76%) va 51-70 (16.59%) yosh guruhlari turadi. Jinslar nisbati 57.56% erkak va 42.44% ayolni tashkil etdi. Eng ko‘p hissani AJK (50.49%), Panjob (27.80%) va Xayber-Paxtunxva (4.15%) qo‘shdi. Klinik va gematologik ko‘rsatkichlar: Taloq o‘lchamini tekshirish yengil (4.63%), o‘rtacha (47.80%) va katta (>8 cm) kattalashishni ko‘rsatdi. 100,000 dan ortiq (31.22%) va 300,000 (17.80%) WBC soni xilma-xillikni ko‘rsatdi. Past gemoglobindan (11.95%) yuqori gemoglobingacha (16.10%). Trombotsitlar soni <150,000 (7.56%) dan >350,000 (37.56%) gacha bo‘ldi. Kasallik fazasi va molekulyar tahlil: Surunkali faza ulushi 92.68%, akseleratsiya fazasi 7.07% va blast fazasi 0.24% ni tashkil etdi. Miqdoriy PCR 88.29% da ijobiy, 2.44% da salbiy natija ko‘rsatdi, 9.27% tekshirilmadi. FISH tahlili 6.83% da ijobiy va 0.49% da salbiy natija ko‘rsatdi, 92.68% tekshirilmadi. Xavf stratifikatsiyasi va davolash: SOKAL shkalasiga asoslangan xavf stratifikatsiyasi 10.73% ni past, 64.63% ni oraliq va 24.63% ni yuqori xavf guruhiga kiritdi. Davolash imatinib (40.49%) va nilotinibdan (59.51%) iborat bo‘ldi. Kuzatuv ma’lumotlari mavjud bo‘lgan 86.83% dan 85.37% da CHR ga erishildi. Xulosa: Ushbu tadqiqot resurslari cheklangan sog‘liqni saqlash sharoitida surunkali mieloid leykozning (CML) turli demografik va klinik xususiyatlarini qamrab oladi. Tadqiqot natijalari cheklangan sharoitlarda CML ni nazorat qilish kasallik bosqichlari, xavf stratifikatsiyasi va davolash rejimlari bilan murakkablashishini ko‘rsatadi. To‘liq gematologik javob (CHR) ko‘rsatkichlari istiqbolli. Ushbu natijalar resurslari cheklangan sharoitlarda bemorlarga ko‘rsatiladigan yordamni optimallashtirish uchun yosh, jins va hududga xos davolash usullarini taklif qiladi. Tadqiqot o‘zgaruvchan demografik va klinik sharoitlarda moslashuvchan usullarning ahamiyatini ta’kidlab, CML ni boshqarish bo‘yicha ilmiy muhokamaga hissa qo‘shadi.
