QUYOSH DERMATITI PROFILAKTIKASIDA ZAMONAVIY TOPIK ANTIOKSIDANTLARNING SAMARADORLIGI

Abstrak

Dolzarbligi. Quyosh dermatiti ultrabinafsha nurlar ta'sirida yuzaga keladigan oksidativ stress, DNK, lipidlar va oqsillarning shikastlanishi natijasida rivojlanadigan, proyallig'lanish sitokinlarining faollashuvi bilan kechadigan terining yallig'lanish kasalligidir. Fotodermatozlar profilaktikasida kislorodning faol shakllarini neytrallash va terining bar'yer tuzilmalarini barqarorlashtirish qobiliyatiga ega topik antioksidantlar alohida ahamiyatga ega. Tadqiqot maqsadi. Fotosezgirligi oshgan bemorlarda quyosh dermatiti profilaktikasida zamonaviy topik antioksidantlarning (vitamin C, vitamin E, ferul kislotasi) klinik samaradorligini baholash. Materiallar va usullar. 60 nafar bemor (Fitspatrik bo'yicha II–III fototip) ishtirokida prospektiv qiyosiy tadqiqot o'tkazildi; ular to'rt guruhga taqsimlandi: A — vitamin C 15 %, B — vitamin E 5 %, C — vitamin C 10 % + vitamin E 2 % + ferul kislotasi 0,5 % kombinatsiyasi, D — nazorat. Ultrabinafsha nurlar ta'sirida yuzaga kelgan eritema jadalligi (erythema index), malon dialdegidi (MDA) darajasi, subyektiv sezgilar (VAS) va quyosh dermatiti epizodlari chastotasi baholandi. Kuzatuv davomiyligi — 8 hafta. Statistik ishonchlilik p < 0,05 da qabul qilindi. Natijalar. Eng yuqori fotoprotektiv samara C guruhida kuzatildi: erythema index 47,30 % ga (p < 0,001), MDA darajasi 41 % ga (p < 0,001), VAS bo'yicha simptomlar 65 % ga (p < 0,001) kamaydi. C va E vitaminlari bilan monoterapiya o'rtacha natijalar berdi (mos ravishda 29,40 % va 21,80 %). Nazorat guruhida eritemaning kamayishi 8,20 % ni tashkil etdi. Quyosh dermatiti epizodlari chastotasi C guruhida eng past bo'ldi — nazoratdagi 11 taga qarshi 2 holat (82 % ga kamayish, p < 0,01). Antioksidantlar kombinatsiyasi yaqqol sinergizmni namoyish etdi va barcha ko'rsatkichlar bo'yicha monoterapiyadan ustun keldi. Xulosa. Vitamin C, vitamin E va ferul kislotasi kombinatsiyasi quyosh dermatitiga qarshi eng samarali profilaktik vosita bo'lib, yallig'lanish, oksidativ stress va subyektiv noqulaylikni sezilarli kamaytiradi hamda klinik epizodlar chastotasini pasaytiradi. Preparatlar yuqori xavfsizlik profiliga va yaxshi chidamlilikka ega.
