Abstract

An analysis of the results of the surgical treatment of 182 patients with pulmonary echinococcosis (of whom 126 had uncomplicated and 56 had complicated pulmonary echinococcosis) is presented. Out of 25 patients, only in 23 was it possible to perform echinococcectomy by a purely thoracoscopic method. In 134 cases videothoracoscopic echinococcectomy was performed through a mini-approach. In 7 patients with bilateral localization of cysts in the lungs, step-by-step operations were carried out, while in 3 (36,36%) cases one-stage echinococcectomy was performed. In 25 patients echinococcectomy from the lung was performed through a wide thoracotomy incision. Anterolateral thoracotomy was performed in all patients. In the majority (97,67%) of cases, organ-preserving operations were performed on the lung, and at the same time the residual fibrous cavity was eliminated in various modifications. In 2 patients with a peripheral location of the cysts and with pneumocirrhosis, the lung tissue was resected together with the echinococcal cyst. 10 patients were operated on for combined echinococcosis of the lungs and liver. In 6 patients surgical interventions on the lungs and liver were performed simultaneously through separate incisions. In pulmonary echinococcosis, minimally invasive methods can be used in more than 2/3 of patients.
