THE IMPORTANCE OF COMPLEX SURGICAL TREATMENT IN INFECTIOUS COMPLICATIONS OF DIABETES MELLITUS

Abstract
The treatment of surgical infections remains one of the main problems of modern surgery. It is known that, owing to the year-on-year increase in the number of cases of diabetes mellitus and of its most dangerous complications, purulent-necrotic complications account for 30-40%. Inflammatory complications of the soft tissues in diabetes mellitus cause sepsis in 44% of cases. In our observation, diabetic foot and purulent-inflammatory complications in the soft tissues were diagnosed in 58 patients with diabetes mellitus. Timely diagnosis is a topical issue in the treatment of such dangerous complications. Diabetic suppuration of the soft tissues and changes in the microvessels in diabetic foot syndrome are examined by means of ultrasound, radiography, Doppler CT and MSCT in order to detect microvascular changes. In patients with diabetes mellitus, microangiopathy develops rapidly. In recent years we have abandoned contrast angiography. All our patients underwent surgical treatment within a short time: the detected phlegmonous abscesses were opened, that is, cleared of necrotic tissues.
