TACTICAL AND TECHNICAL ASPECTS OF VENTRAL HERNIA SURGERY

Abstract
The treatment results of 107 patients with postoperative ventral and recurrent hernias were studied, who underwent repair of the hernial defect using onlay and sublay methods, as well as using separation repair with restoration of normal topographic anatomy. The results of perioperative monitoring of intra-abdominal pressure at W3-W4 ≥ 11 justify the priority of posterior separation hernioplasty; at ˂11 anterior separation repair may be performed. The algorithm for choosing the method of repair - standard (onlay, sublay) or separation (anterior, posterior) - in postoperative ventral hernias W2, W3, W4 is based on the condition of the musculo-aponeurotic structures of the anterior abdominal wall and on the indicator of intraoperative monitoring of intra-abdominal pressure.
