O‘TKIR ISHEMIK INSULTDA ENDOVASKULYAR REPERFUZIYA VA ERTA REABILITATSIYA: TASVIRLASHGA ASOSLANGAN INDIVIDUAL YONDASHUV

Abstrak

O‘tkir ishemik insult dunyo bo‘yicha o‘lim va uzoq muddatli nogironlikning asosiy sabablaridan biri bo‘lib qolmoqda. To‘qima plasminogen aktivatori bilan intravenoz tromboliz insultni erta bosqichda samarali boshqarishni sezilarli darajada yaxshilagan bo‘lsa-da uning samaradorligi tor terapevtik oynasi va katta tomir yopilishlarida to‘liq rekanalizatsiya bo‘lmasligi bilan cheklangan. So‘nggi paytlarda endovaskulyar reperfuziya texnikasidagi ilg‘or yutuqlar davolash sohasini tubdan o‘zgartirdi, bu esa tomirlarning muvaffaqiyatli rekanalizatsiya darajasini oshirish va klinik natijalarni yaxshilash imkonini berdi. Zamonaviy yondashuvlar jumladan, mexanik trombektomiya, aspiratsion trombektomiya va intraarterial tromboliz, ayniqsa intravenoz trombolitik terapiya bilan birgalikda qo‘llanganda yuqori samaradorlikni namoyon qilmoqda. Shuningdek, yangi fibrinolitik vositalarning rivojlanishi va ultratovush yordamida tromboliz kabi yordamchi usullar terapevtik imkoniyatlarni yanada kengaytirdi. Reperfuziya strategiyalari bilan bir qatorda, o’tkir va akut bosqichda reabilitatsiyani erta boshlash nevrologik tiklanishni yaxshilash, neyroplastiklikni rag‘batlantirish va uzoq muddatli nogironlikni kamaytirishda muhim rol o‘ynaydi. Ilg‘or endovaskulyar aralashuvlarni erta reabilitatsiya bilan integratsiyalash o’tkir ishemik insultni boshqarishda kompleks va bemor markazli yondashuvni tashkil qiladi. Kelajakda individual, tasvirlashga asoslangan davolash strategiyalari va ko‘p tarmoqli yondashuvlar funksional natijalarni optimallashtirish va terapevtik oynalarni kengaytirishga e’tibor qaratadi.
