QIZIL YASSI TEMIRATKINING TURLI SHAKLLARIDA DERMATOSKOPIYANING DIAGNOSTIK AHAMIYATI

Abstrak

Dolzarbligi. Qizil yassi lishay (QYL) yaqqol klinik polimorfizmga ega geterogen immun-yallig'lanish kasalligi bo'lib, bu o'z vaqtida tashxis qo'yishni va uning shakllarini farqlashni sezilarli qiyinlashtiradi. An'anaviy biopsiya «oltin standart» bo'lib qolmoqda, biroq uning invazivligi va shilliq qavatlar zararlanganda cheklangan qo'llanilishi noinvaziv usullarni rivojlantirishni talab qiladi. Yallig'lanish, pigmentatsiya va tuzilmaviy o'zgarishlarning subklinik belgilarini vizualizatsiya qilish imkonini beruvchi dermatoskopiya QYL ni erta tashxislashda asosiy vositaga aylanmoqda. Dermatoskopik mezonlarning yetarli standartlashtirilmagani va ularning kasallikning turli shakllarida o'zgaruvchanligi tashxis aniqligini oshirish hamda bemorlarni klinik olib borish taktikasini optimallashtirish maqsadida dermatoskopik patternlarni yanada o'rganish zaruratini belgilaydi. Tadqiqot maqsadi. Dermatoskopiyaning diagnostik ahamiyatini baholash va qizil yassi lishay (QYL) ning turli klinik shakllariga xos dermatoskopik belgilarni aniqlash. Materiallar va usullar. Tadqiqotga klinik tasdiqlangan QYL bilan 84 bemor kiritildi. Klinik ko'rik, raqamli dermatoskopiya (HandyScope, DermLite DL4), dermatoskopik patternlarni IDS mezonlari bo'yicha baholash, shuningdek 42 bemorda tashxisning morfologik verifikatsiyasi o'tkazildi. Statistik tahlil χ² va t-test ni o'z ichiga oldi (p<0,05). Natijalar. QYL ning klinik shakllari o'rtasida dermatoskopik patternlarning yaqqol farqlari aniqlandi. Vikxem chiziqlari papulyoz shakldagi bemorlarning 94,7 % ida, pigmentli shaklda esa atigi 20 % ida qayd etildi; boshqa shakllarda bu belgi kuzatilmadi. «Peppering» turidagi pigmentatsiya va diffuz to'q qo'shilmalar pigmentli (100 %), follikulyar (71 %) va papulyoz (71 %) shakldagi bemorlarning ko'pchiligida qayd etildi. Eroziv-yarali shakl yaqqol qon tomir arxitekturasi bilan tavsiflandi. Olingan ma'lumotlar QYL ning turli shakllarini tashxislashda dermatoskopik belgilarning yuqori o'ziga xosligini namoyish etadi. Xulosa. Dermatoskopiya QYL da tuzilmaviy o'zgarishlarni vizualizatsiya qilishning yuqori axborotli noinvaziv usulidir. Aniqlangan patternlar tashxisni standartlashtirish, klinik shakllar o'rtasidagi farqlashni yaxshilash va, ayniqsa tipik hamda yaxshi ko'rinadigan holatlarda, teri biopsiyasiga bo'lgan ehtiyojni kamaytirish imkonini beradi. Dermatoskopiya qizil yassi lishayni tashxislashda birinchi qator usuli sifatida ko'rib chiqilishi kerak.
