THE CHOICE OF TREATMENT METHOD FOR PATIENTS WITH POSTOPERATIVE VENTRAL HERNIAS AND OBESITY

Abstract
One of the reasons worsening the results of hernioplasty is obesity. The relevance of the problem of surgical correction of abdominal deformities in overweight patients is due not only to the large number of complications of the early and late postoperative periods and of relapses, but also to the uncertainty of tactical and methodological approaches. Patients with morbid obesity also have high intra-abdominal pressure compared with patients without obesity. The aim of the work is to improve the treatment results and the quality of life of overweight patients suffering from midline ventral hernias by combining hernioplasty with reconstructive interventions on the anterior abdominal wall. The work is based on the study of the results of surgical treatment of 73 patients with ventral hernias against the background of excess body weight. In all cases, tension-free hernioplasty using mesh explants was performed. The following were chosen as comparative criteria of the course of the early postoperative period in the two groups of patients: the severity of the pain syndrome, the time of restoration of intestinal function, the duration of the operations, the presence of complications, the assessment of the functional state of the muscular apparatus of the anterior abdominal wall, and the average length of hospital stay. An increase in the volume and duration of hernioabdominoplasty does not lead to an increase in the frequency of either early or late postoperative complications. Simultaneous hernioabdominoplasty is a pathogenetically justified surgical intervention in overweight patients with abdominoptosis suffering from ventral hernias.
